Train Mind Body Soul – Informed Consent Form

1. Programme Objectives

You are participating in either one-to-one personal training or small-group Strength Camp sessions. These are designed to improve strength, fitness, and overall wellbeing through structured physical activity.

2. Potential Risks and Benefits

Risks: may include musculoskeletal injury, cardiovascular incident, or physical discomfort.
Benefits: improved strength, fitness, mobility, and wellbeing.

3. Your Responsibilities

- Disclose all medical conditions and medications.
- Complete a PAR-Q and update it when your health status changes.
- Stop exercise and inform your trainer if you experience pain, dizziness, or discomfort.
- Follow all instructions from your trainer, particularly when using equipment or performing exercises.
- Ensure a safe environment if participating in online sessions.

4. Consent Statement

I confirm that I understand the objectives, risks, and responsibilities involved in participating in this programme. I acknowledge that participation is voluntary and I may withdraw at any time. I understand that my trainer is not a medical professional and cannot diagnose or treat medical conditions. I consent to participate in training with Train Mind Body Soul.
